WE RESERVE THE RIGHT TO REFUSE ADMITTANCE. By submitting the above application, I do hereby hold harmless the Milliet Academy of Dance, the teachers, assistants and associates responsible for any injuries to my child while in class or on the premises of the property. I realize that I have full responsibility for my child prior to and immediately after his/her class. I understand that tuition is figured on a monthly basis payable by the 10th of each month. A $15.00 late fee will be added to accounts not paid by the 10th of each month. No refunds or deductions are given if a child misses for any reason. I also understand that tuition, recital fees and costume deposits are non-refundable. I hereby give permission for the Milliet Academy of Dance to take photographs for promotional uses for the studio.
